Membership Application

Important:

• Through December 31, 2025, there is no application fee.

• Beginning January 1, 2026, a non-refundable application fee will be required before the form can be submitted.

• All applicants, regardless of membership tier, must pay an application fee beginning in 2026.

• Application submission does not guarantee acceptance into the OLPS Members Group.

• All applications undergo manual review by the OLPS Board.

• Set aside enough time to complete in one setting. Your progress will not be saved if you come back later.

If accepted as a member, you will be required to complete a public profile including a professional headshot, CV, social media links (if applicable) and links to your current practice/hospital where patients and fellow members could find you.
